Mediastinitis (deep sternal wound infection) is a rare but life-threatening complication for our patients after median sternotomy in cardiac surgery. The introduction of the vacuum therapy has achieved a tremendous improvement in the outcome for the treatment of deep sternal wound infections. The mortality rate of 20%–25% undergoing conventional therapy (continuous irrigation with drainage and wound closure with the use of the greater omentum) is now reduced to less than 10% using vacuum therapy. Anyway, there are a lot of different therapeutic procedures varying from clinic to clinic for the treatment of mediastinits. In an optimal matter the treatment should be achieved in a multidisciplinary cooperation of cardiac and plastic reconstructive surgeons.

The working group wound management of the German Society for Thoracic and Cardiovascular Surgery implemented a nationalwide Mediastinitis Registry on June 2011, which is directly accessible via the link on the homepage of our society. The goal of the working group is to evaluate prevention strategies for surgically associated wound infections to reduce their occurrence and to develop specific algorithms for the treatment of wound infections.

Therefore we need a complete data access for a different analysis of all various therapeutic strategies for the treatment of mediastinitis. We invite the DGPRÄC (Deutsche Gesellschaft der Plastischen, Rekonstruktiven und Ästhetischen Chirurgen) and the VDÄPC (Vereinigung der Deutschen Ästhetisch-Plastischen Chirurgen) to an active participation at the Mediastinitis Registry to achieve a multidisciplinary cooperation in the treatment of deep sternal wound infections. The German Mediastinitis Registry allows a detailed statistical analysis of patient data in order to develop and optimize improved prevention and treatment strategies (Figure 1 [Fig. 1], Figure 2 [Fig. 2]).
