Objective
Evaluation of the usefulness of fMRI for presurgical planning regarding language and motor cortex.
Methods
BOLD fMRI in 11 healthy volunteers and 30 patients with brain tumor, intracerebral AVM and temporal lobe epilepsy were performed on a 1.5T MRI system (Siemens Symphony with Numaris 3.5 VA13G) using gradient echo EPI sequence. The subjects were studied during the following different conditions: finger tapping, tactile activation, continuous speech and word generation tasks. fMRI data were subjected to a pixel-by-pixel analysis of signal intensity differences between images at the rest and the task period, using the software of the MRI equipment.
Results
Finger tapping showed activation in all subjects. Language tasks showed only slight activation in some patient and there was no significant activation in one patient. In some patient we were able to localize the dominant hemisphere giving additional information about the localization of eloquent areas. Movement artefacts caused poor quality results in some patients.
Conclusions
BOLD fMRI is a useful technique to detect cortical activities on 1.5T MRI system. The clinical value of the method is high, it is accepted by the neurosurgeons, but motion artefacts can make annoying difficulties.
