First approaches to accommodating intraocular lenses were not successful. One the main failure causes was capsular fibrosis that rapidly prevented any IOL movement or change of curvature.

In 2000 we introduced the Concept 360 IOL, aiming at preventing lens epithelial cell migration, proliferation and fibroblastic transformation. Experimental and clinical results were significant in such a prevention, mainly due to an absence of contact between the anterior capsule and the IOL optic, creating the first open bag system which has paved the way to capsular fibrosis prevention.

Since the beginning of the century, the open bag approach has been used with different devices, single and double optics, circular rings compressing soft optics, all based on capsular adhesion prevention.

We present here the initial experimental results on rabbits of a new concept to prevent capsular fibrosis. Through a 2.3 mm incision we introduced a circular device attached to the optic. At 6 weeks, the capsules, anterior and posterior, remained clear with minimal Sommering’s ring formation. This capsular clarity and suppleness could allow us to develop an accommodating system.
