Aqueous shunts are widely used in glaucoma recalcitrant to other forms of treatment. Randomised clinical trials have shown implants such as the Baerveldt 101-350 to be capable of achieving IOP levels in the low teens in situations where conventional filtration surgery is either impossible or unlikely to achieve effective control.
Implantation of these devices is not without risk, including complications such as those related to hypotony, exposure, corneal endothelial loss and motility disturbances. A number of trials have examined the role of shunts in patients with failed filtration surgery, prior cataract surgery and even those with no previous intraocular surgery. In addition the most commonly used shunts, the Ahmed Glaucoma Valve and the Baerveldt 101-350 have been compared in 2 separate trials.
This talk will outline the evidence for the use of shunts for differing indications, the risks and some techniques that facilitate successful implantation and outcomes as well as to look forward at some future possibilities.
