30 years exposure to a population group with amongst the highest incidence of glaucoma in the world has revealed that timing of decisions in the management of glaucoma to be paramount.

The relentless progression of structural and functional damage caused by glaucoma is much faster than in the European population.

Careful evaluation by observation using very high magnification and definition has yielded the following facts :

Trabecular changes
2.
Canal of Schlemn collapse
3.
Collector channel atrophy
4.
Relentless increase of IOP

In a population where we have found the mean IOP at the time of surgery to be 47 min Hg. It becomes very obvious that early detection and early surgical intervention make for the best prognosis and results. These findings will be supported by follow-ups as longs as 25 years using very high quality videos to emphasize the timing of surgical intervention.
