Small or poorly dilating pupils have always been a challenging situation for cataract surgeons. The strategies for pupil enlargement greatly depend on surgeon skill and preferences, as well as on the intraoperative situation. In patients with insufficient mydriasis observed at the beginning of the procedure, I recommend using the step-wise approach starting with intracameral injection of unpreserved phenylephrine, followed by posterior synechiolysis, gentle iris stretching and then use the mechanical pupil expander. Malyugin Ring (produced by MST, USA) is the new pupil expander having a square shape and 4 circular curls located at the angles of the device. It provides balanced stretching and gentle holding of the iris tissue. In the presentation different surgical techniques with the Malyugin Ring in variable clinical situations will be described.
