A recent classification of the Glaucomas by Lucio Burrato came up with 62 sub-divisions which makes for a very complicated understanding of the disease at first glance. To simplify this concept an understanding of the micro anatomy, physiology and pathology of the disease can be beautifully demonstrated with photographic techniques developed over many years where the 4 cardinal structures are revealed at magnifications of up to 250×. These structures are

- Canal of Schlemm
- 2.
- Trabecular Meshwork
- 3.
- Collectors Channels
- 4.
- Optic nerve of N.F.L.
Very high magnification in real time reveals the entire mechanism and cycle of the disease glaucoma. We have resolved that micro anatomical appreciation indicates the following important points.
- 1.
- The Trabecular meshwork ………. Is it permeable or non-permeable?
- 2.
- The Canal of Schlemm ………. Is it patent or collapsed?
- 3.
- The Collector Channels ………. Collapsed or patent?
Permutations of these 3 entities determine the prognosis.
