Introduction: The human cystic echinococcosis is a zoonosis transmited by the ingestion of E. granulosus eggs eliminated by infected dogs. Uruguay is one of the endemic areas with the highest rates of infection, 67% in sheep and near 1.3% in humans in rural areas.

Materials and methods: 47 year-old man with a three-month history of productive cough, dyspnea and weight loss, his sputum was green and copious.

A diagnosis of complicated hydatic cyst with bronchobilary fistula and echinococcal neumonia was made.

Results: Right lower lobectomy (via thoracotomy) and hepatic pericystectomy (via laparotomy) was performed.

Conclusion: Although this pathology is endemic in our country, bronchobiliary fistulas are not very common. The outcome from this condition is usually good if rapidly identified and treated, as it happened in our patient.
