Surgical management of expansive parapharyngeal space tumors in childhood

Tumors of the parapharyngeal space are overall rare and account for approximately 0,5% of total head and neck neoplasms. The neurovascular anatomy of this potential space is not only dense, but also vital; therefore intraoperative damages may lead to serious morbidity even mortality. Obtaining access of this sensitive, deep-seated, pathology has been always a challenge. In cases of benign laesions – beside the complete removal – we have to reach preserved function and achive optimal aesthetic result. These goals are highly influenced by the appropriate visualisation of the surgical site. Various surgical approaches have been described. The choice of the surgeon is influenced by the exact localisation, size and histopathology of the tumor, and by the age of the patient. The authors present the different surgical approaches of the parapharyngeal space, and through three cases they specify the transcervical removing of a myo-fibroma, the transoral removing of a ganglioneuroma, and the facial degloving technique by a retromaxillar myxoma. The endoscopic-assisted techniques reduced radically the size of the exploration in all three cases, and with them a much more sufficient aesthetic result was available.
