Proper placement and fixation of spreader grafts in rhinoplasty is frequently quoted as an indication for the open structure approach. Alternatives to suture fixation may be associated with compromised precision and may increase the risk of migration or displacement. The autors present a technical algorithm to facilitate the endonasal placement and suture fixation of spreader grafts through the endonasal approach in rhinoplasty. The technique is shown in detail and results of 100 consecutive patients are analyzed.
