Total nasal reconstruction presents multiple challenges. Accepted concepts include a staged approach, formation of a skeletal framework with costal cartilage and epithelial cover with a paramedian forehead flap. Reported techniques of generating internal lining seem more variable and include epideral turn-in flaps and a variety of endonasal mucosal flaps. The morbidity associated with endobasal mucosal transfer is generally accepted as it is outweighed by the reconstructive benefits. We present a therapeutic algorithm designed to minimize the use of endonasal mucosal transfer and to maximize endonasal physiologic function in patients undergoing total nasal reconstruction. Techniques are presented in detail and results are demonstrated.
