A 19-year-old boy with severe aplastic anemia e.c.i., was diagnosed with Rhizopus oryzae (mucormycosis) in the frontal sinus. As a very aggressive and often mutilating disease, rhino-orbital-cerebral mucormycosis is notorious for its poor prognosis with up to almost 100% mortality.

We present a patient with successful treatment and excellent aesthetic result. The success of the treatment was to our opinion rapid diagnosis and long-term systemic treatment with Amphotericin B, in combination with the intensive local therapy. This local therapy consisted of nasendoscopically guided rinsing with 5 cc Amphotericin B dissolved in 20 cc NaCl, combined with multiple endoscopic sinus surgery (including Draf2A with places endonasal drains, Draf 3, revision Draf3 with external frontalis drains) in order to obtain optimal drainage of the frontal sinus.
