Successful hearing restoration in revision stapes surgery for otosclerosis requires precise identification and correction of the respective abnormality without traumatizing the inner ear. Conventional surgical procedures often lead to unsatisfactory hearing results and inner ear lesions.
In revision surgery, the CO2 laser provides ear surgeons with three important advantages over the conventional technique: 1. improved diagnostic and therapeutic precision by bloodless ablation of connective tissue and bone, 2. reduced inner ear trauma through the protective noncontact procedure for removing old prostheses, and 3. better possibilities for stabilizing the new prosthesis in the oval niche.
The surgical technique in revision stapedotomy with the CO2 laser is presented and the possibilities of the laser surgery in different pathologies (connective-tissue and/or bony reobliteration of the oval niche, fractured footplate, mobile footplate etc.) is discussed.
Our revision stapedotomies with the CO2 laser have yielded better postoperative hearing results than with the conventional technique and have involved no significant sensorineural hearing losses. The CO2 laser enables high-precision and safe correction of a conductive deafness relapse after stapes surgery.
