Long-term tumor control of spinal dissemination of cerebellar glioblastoma multiforme by combined adjuvant bevacizumab chemotherapy

Objective: In this case study, we report of a female patient with an exophytic glioblastoma multiforme (GBM) arising from the cerebellar tonsil and a secondary spinal manifestation. Furthermore, the tumor showed no MGMT promotor-hypermethylation and no IDH-1 mutation. All these signs are accompanied by significantly shorter median overall survival.
Method: The patient's clinical course was documented while undergoing combined temozolomide/bevacizumab and irradiation therapy.
Results: A long-term tumor control of the spinal metastases over 33 months was achieved by a combined temozolomide/bevacizumab and irradiation therapy. To our knowledge this is also the first published case of a combined cerebellar exophytic GBM with a subsequent solid spinal manifestation.
Conclusions: This case supports a critical re-evaluation whether bevacizumab might be a useful agent in the treatment of glioblastoma cases with symptomatic disseminated spinal manifestation.
