Purpose: Aim of the presentation is to show a case series of cosmetic IOL implant (Ophtec REF 311) in partial or total post-traumatic aniridia.

Methods: A PMMA unfoldable IOL. Our surgical technique: two scleral flaps at 3 and 9, use of prolene 10-0 suture, endoacopic control; 25 gauge anterior infusion. If possible the IOL can be fixed on the remains of the anterior capsule.

Results: We performed the implant on 28 patients: 27 scleral fixed and 1 in the sulcuson the anterior capsule. No intra-operative complications. During the follow up, 4 cases presented anterior and intermediate uveitis. Follow-up 6 - 48 months.

Conclusions: The Ophtec REF 311 IOL is a valid solution in terms of anatomic, hestetic and functional results in the treatment of post-traumatic aniridia and aphakia. The endoscopy can be helpful during the operation to ceck the surgical steps.
