Tumours of the parotid gland are of low incidence in Germany. So far there exist only a few data on risk factors for the development of parotideal tumours and salivary gland tumours in general.

In the setting of a case-control-study 366 patients, in whom a parotideal tumour was resected in between 2001 and 2005, and 366 control persons were examined. By the help of a standardized and computer legible questionnaire, the habits of living, job-associated and social factors were registered as well as concomitant diseases.

In 86% of the patients a benign tumour and in 14% a malign tumour was found. Pleomorphic adenomas (45%) were 1.5 fold more frequent in women than in men. In contrast to this cystadenolymphomas (23%) were 3.3 fold more frequent in men than in women. Distant metastases of other primary tumours represented 49% of all malignant parotideal tumours. Among these the adenoid-cystic carcinoma played an important role.

Cystadenolymphomas were associated with smoking. Otherwise no statistical significance was shown between tobacco and alcohol consumption as well as nutrition habits versus the risk of developing a parotideal tumour. Significant was an accumulation of parotideal tumours in industrial areas (5.0 per 100:000 versus 3.5 per 100:000 in rural areas).

Pleomorphic adenomas were found by preference in younger patients (median 52 years) in comparison to malignant tumours (median 66 years). Summing up, this so far largest epidemiologic study performed in a German patient collective merely showed hints for a different distribution in age and sex as well as a raised incidence rate in industrial areas.
