Introduction: Eradication of inflammatory diseases of the external ear canal, the middle ear and mastoid cavity and the lateral skull base often necessitates the restoration of the complex anaomy. Bone chips from the temporal squama or the mastoid cortex and/or bone dust, which is obtained during the drilling procedure is very suitable for reconstruction of various areas of the temporal bone.

Methods: In this review the indication and application of autogenous bone grafts in different regions of the temporal bone is demonstrated and discussed::

1.
Reconstruction of the posterior-superior canal wall
2.
Reconstruction of the lateral attic wall
3.
Reconstruction of the anterior canal wall in temporomandibular joint fistulas
4.
Obliteration of the epitympanic recess
5.
Coverage of semicircular fistulas/mastoid cavity obliteration
6.
Reconstruction of tegmen tympani defects in meningoencephaloceles
7.
„Bone shoe“ for TORP-protheses/footplate reconstruction

Results/Discussion: Autogeneous bone is an excellent material for reconstruction in middle ear and skull base surgery, because it offers intrinsinc stability, the advantage of complete ingrowth into the bony surroundings, good viability and the easy harvest.
