Phakic intraocular lenses (PIOL) are gaining interest because of increasing limitations of laser surgery and improved outcomes after PIOL implantation.

Design modifications further decreased the complication rate. Nevertheles, serious concerns and risks (e.g. endophthalmitis, cataract, progressive endothelial cell loss) and open questions remain, requiring longterm follow-up and examination of patients after PIOL implantation. Various types of PIOL with their respective fixation location will be introduced (angle, iris, posterior chamber) including indication, surgical technique and refractive results( predictability, stability, safety). Anatomic and pathophysiological considerations will performed. New indications, own experience and new lens designs, which hold great promise, are to be discussed.
