Objective

For treatment of a high myopia and astigmatism a toric ICL was implanted, but a few hours after surgery patient presented with an extremely high intraocular pressure and shallow anterior chamber because of a sizing problem.

Methods

The toric ICL had to be explanted emergency and an iris-fixated phakic intraocular lens was implanted in the same surgical session.

Results

Intraoperative course was difficult.

Conclusions

This video demonstrates surgical proceedings and decisive steps to solve intra- as well as postoperative problems. Solutions to prevent situations like this are provided.
