The treatment of the flat foot in the young and adolescent belongs to the most frequent diseases in children in orthopedics outpatient clinic. Common treatment regimes are physiotherapy as well as special inlays and orthesis. The operative correction of the hindfoot is indicated in therapy resistent, painful and severe deformation.

Traditional operations, like the Grice-procedure (1952), are irreversible and may result in overcorrection. Tendon transpositions, like the Niederecker-procedure, have no influence on the bony deformity. In contrast, reversible operations, like the calcaneus stop procedure, are true alternative.

The MBA screw is used for this operation. It has been introduced in 1995 and is frequently used in our clinic to treat the skew-weak foot. This canulated titan screw for the subtalar arthrorisis is used for the correction of the flexible hindfoot pronation in the sence of a guidance aid with preservation of the function of subtalar joint (calcaneus stop procedure). The implantation is reversible everytime without major hardware.

Since February 2003, 6 MBA implantations have been performed in 5 patients age 10 to 14 years. The postoperative position of the hindfoot showed always good results. The rehabilitation was quick. The subjective and objective parameter revealed a functional improvement immediately after the operation.

Although long term results are not yet available, our own experience and the results of other groups indicate the use of the MBA screw for the treatment of the skew-weak foot.
