Different influence of an ACEI/Calciumantagonist combination on the pulse pressure in comparison to the monotherapy

The pulse pressure can be regarded as the clinical correlate to the elasticity of the arteries measured by pulse wave velocity. Based on datas of the Framingham study the pulse pressure is accepted as an independend risk factor on the cardiovascular morbidity especially in older hypertensive patients. On the other hand the influence of various antihypertensive regimes on the pulse pressure is relatively unknown.
Therefore we investigated two substances with well known positive effect on the vascular structure, the ACEI ramipril (ram 5 mg), the calcium channel blocker felodipin ER (fel 5 mg), and the combination of both (D = ram + fel 5/5) with regard to the effects of the systolic and diastolic blood pressure (BP) and the pulse pressure in a prospective randomized multicenter study of 150 hypertensive patients. BP was measured in the office and at home by self BP measurements (7 fixed time points/d).
The effects of these different antihypertensive treatments are shown in Table 1 [Tab. 1].
Conclusion: During 4-week-treatment the fixed dose combination as compared to its individual components was significantly more effective in reducing systolic and diastolic blood pressure especially in reducing the pulse pressure
