Trough videofilm are demonstrated the main steps of surgical method for recovery the integrity of tympanic membrane, applicable in total and subtotal tympanic defects, associated with chronic otitis media – mesotympanitis.
The method has been used in the ORL Department in Medical University – Sofia, over the last 30 years with very good morphological and functional results. So far, 900 patients have been operated using this technique.
As a rule, the surgical procedure is performed under local anesthesia. Through endaural incision a composed graft of cartilage and overlaying perichondrium is harvested from tragus.
Preparation of a vascular bed, modeling and adapting the graft over the skeletonized malleus handle - promptly fixed into the tympanic annulus with the perichondrial end, is subsequently carried out.
Graft surface is covered with a silastic protector, followed by packaging of the ear canal with gelfoam. After 2 weeks, the ear packaging is removed.
The late results show that in over 90% of cases has been established recovery of the tympanic membrane and hearing normalization.
