Papillary carcinoma in a thyreoglossal duct cyst: ectopic primary or metastasis?

Thyreoglossal duct cysts are the most common anomaly in thyroid development. The occurrence of carcinoma in a cyst is reported to be about 1%. Worldwide about 200 cases are described. In about 85% histopathology reveals a papillary thyroid carcinoma. In order to elucidate the therapeutical management of such a mostly incidentally diagnosed carcinoma we report the case of 38-year-old female that presented with a cystic cervical mass. After Sistrunk procedure an ectopic papillary thyroid carcinoma was diagnosed. There was no evidence for a malignant tumor in preoperative Tc-99m-Scan, MRI and ultrasound. A total thyoidectomy and bilateral neck dissection was performed. This revealed a small thyroid carcinoma and 3 positive lymphatic nodes. Postoperatively we carried out a radioiodine therapy and a percutane radiotherapy. The histopathological findings support our relatively radical therapy. Weather the cystic carcinoma is the primary or a metastasis of the thyroid carcinoma is a matter of debate. Inndeed the existence of two independent primary carcinomas (multicentric) could be possible.
