Persistent measles virus infection of the otic capsule is supposed to be one of the etiologie factors in otoclerosis. Viral pathogenesis of otoselerosis could be established only by corrclation analysis of stapes footplate histology and RT-PCR amplification of viral RNA. Nucleic acids (mRNA,vRNA, DNA) were extracted from stapes footplate samples of clinically otosclerotic patients. The genomic nuclcoprotein RNA of measles virus was amplified by semi-nested RT-PCR. PCR results were correlated to postoperative histologie findings and to preoperative audiometric examinations. Among 10 histologie specimens of clinically otosclerotic patients, measles virus RNA was detectable only in historlogically confirmed otoselerotic stapes footplates (N-5). Virus negative stapes footplate samples(N-5) showed annular calcification (N-3), bone resorption with increased number of hacmosiderophages (N-1) and mononuclear cell infiltration with ostcolysis (N-1) Stapes ankylosis is a heterogenous disease causing conductive hearing loss with different etiologie. Non-otosclerotic stapes fixations are degenerative disorders. Otosclerosis is an inflammatory discase due to persisting measles virus infection of the otic capsule. Preoperative, audiological differential diagnosis of otocscelerotic stapes fixation seems possible on the basis of presence of the Carhart's notch. Non-otoselerotic stapes fixation is not featured by this audiological phenomenon.
