Einleitung: Die pneumologische Rehabilitation ist integraler Bestandteil der therapeutischen Maßnahmen bei vielen chronischen Lungenerkrankungen und ihre unmittelbare Effizienz ist wissenschaftlich gut belegt.

Ziel dieser systematischen Literaturanalyse ist die Beantwortung folgender Fragen:

  • Welche Effekte einer pneumologischen Rehabilitation lassen sich auch noch nach längeren Zeitabständen nachweisen?
  • Wo besteht weiterer Forschungsbedarf?

Methodik: Die Studiensuche erfolgte über „Medline“. Ausgewählt wurden die Studien aus dem Zeitraum 1990-2006, in denen Nachuntersuchungen mit Evaluation von in den Evidenzgraden A-C angegebenen Parametern mit einem zeitlichen Anstand von mindestens 3 Monaten nach der Rehabilitation durchgeführt worden sind.

Ergebnisse: Für die Rehabilitation bei COPD können auch nach einem Jahr noch signifikante Verbesserungen von Lebensqualität und der 6-Minuten-Gehstrecke als Ausdruck der körperlichen Belastbarkeit sowie eine Reduktion der COPD-bedingten Krankenhausaufenthalte dokumentiert werden.

Patienten mit interstitiellen Lungenerkrankungen werden nur selten einer pneumologischen Rehabilitation zugeführt. Die bisherigen Daten aus aktuellen Studien zeigen neben sofortigen Effekten auf Lebensqualität und Gehstrecke, dass auch langfristige Verbesserungen möglich sein könnten.

Berufsbedingte Lungenerkrankungen wie Silikosen und Asbestosen stellen sich oft als komplexes Mischbild mit bronchialen und parenchymbedingten Erkrankungsteilen dar, deren Rehabilitation bislang nur durch eine Studie evaluiert wurde, welche sowohl direkt nach der Intervention als auch nach 3 Monaten eine signifikante Verbesserung der Lebensqualität und Leistungsfähigkeit zeigt.

Schlussfolgerungen: Die Entscheidung über die Form der Rehabilitationsmaßnahme, ambulant oder stationär, unterliegt praktischen Kriterien wie örtlichen Gegebenheiten und den persönlichen Lebensumständen. Deshalb sollten die beiden Formen nicht als konkurrierend, sondern als sinnvolle Ergänzung zueinander gesehen werden.

Es sind weitere gut angelegte Studien mit größeren Kollektiven notwendig, welche den nur schwer in einzelne Parameter zu teilenden Gesamteffekt der pneumologischen Rehabilitation evaluieren können.

Unser Institut führt derzeit in Zusammenarbeit mit den berufsgenossenschaftlichen Kliniken Falkenstein und Bad Reichenhall eine Studie mit 320 Patienten mit anerkannten Berufskrankheiten der Atemwege oder der Lunge (Asbestose, Silikose) zur Evaluation der Langzeiteffekte bis maximal ein Jahr nach Rehabilitation durch.
