Objective

We report an original case of progressive paraparesia in adult revealing an interhemispheric arachnoid cyst.

Methods

We admitted a 73-year-old female who was operated for lumbar stenosis 4 years ago, and presented a progressive paraparesia. Because of recent headache cerebral CT Scan and MRI were realized. They revealed an interhemispheric arachnoid cyst which was removed by an interhemispheric approach.

Results

Postoperatively the neurological signs and symptoms were markedly improved and the size of the cyst was greatly reduced.

Conclusions

The interhemispheric fissure is an uncommon site for benign cyst. To our knowledge, less than 12 cases of symptomatic interhemispheric arachnoid cyst in adult have been reported. The diagnosis, mechanism of symptom onset and management of symptomatic interhemispheric arachnoid cyst in the elderly is dicussed.
