Background: The Anti-VEGF-therapy represents the standard therapy in the treatment of exsudative AMD. During the treatment, however, there can be observed very diverse reactions and changes in the morphological parameters. Therefore we analysed the morphological changes in the Spectral-Domain OCT during the Anti-VEGF-therapy about 28 months.

Method: We recorded data of 93 eyes (ĝ 78,0 y) with a minimum follow-up of 12 months (medium follow up of 80 weeks), which were treated with an injectiontherapy of Ranibizumab. Besides the clinival examination we carried out a Spactral-Domain OCT before and after treatment. In the latter we differentiated initially and in progress the of the treatment (0–108 weeks) the presence of intra- as well as subretinal liquid, the bigness of the cystoids cavities, the height of the subretinal liquid, the thickness of the central retina and the continuity of the photoreceptorjunction zone.

Results: The presence of intra- and subretinal liquid as well as the bigness of the cystoid cavities and the height of the subretinal liquid decreased in the progress. The cystoids cavities in the outer retinal layers regressed more than the cavities in the inner retinal layers. The photoreceptorjunction zone was initially interrupted mostly. In a few cases this zone regenerated completely. In the progress the length of the interrupted photoreceotorjunction zone reduced in many cases. The thickness of the central retina reduced, too.

Conclusion: If one compares various parameters in the Spectral-Domain OCT during the treatment with Ranibizumab, it shows that many parameters decrease considerably, like e.g. the intra- and subretinal liquid, the bigness of the cystoids cavities as well as the height of the subretinal liquid and the thickness of the central retina. We noticed that the cystoids cavities in the outer retinal layers regressed more often than the cavities in the inner retinal layers. The various changes seemed to be important for the re-treatment. On the other hand the partial or complete regeneration of the photoreceptorjunction zone is important for the prognosis of the visus.
