Background: Retinal pigment epithelial rips in children are very unusual and have not been further reported so far.

Case report: A twelve-years-old girl suffered from inner ear deafness and retinopathy presumably due to asymptomatic intrauterine varizella-zoster-infection. When she presented first with visual loss in the OD, a minimal classic CNV was diagnosed. At that time, no signs of retinal pigment epithelial rip were found. After written informed consent was obtained from her parents, the child was treated by three consecutive intravitreal injections with ranibizumab 0.05 mg. Three weeks after the first injection, a retinal pigment epithelial rip was found. The two following injections were continued as predetermined. Visual acuity improved from 20/400 to 20/50. Optical coherence tomography showed significant reduction of intraretinal and subretinal fluid.

Conclusions: A retinal pigment epithelial rip which is very unusual for children developed three weeks after intravitreal injection of ranibizumab. Improvement of visual acuity however occurred after two further injections as previously reported in adults.
