Arg389Gly beta1-adrenoceptor (AR) polymorphism exhibits decreased receptor signaling in vitro.

We studied in 10 male subjects homozygous for Arg389 beta1-AR and 8 male subjects homozygous for Gly389 beta1-AR whether beta1-AR mediated plasma-renin-activity (PRA)-increase and beta1-AR mediated hemodynamic effects in vivo are also affected by beta1-AR polymorphism; to avoid influences of codon 49 polymorphism all subjects were homozygous for Ser49. Subjects were infused with dobutamine (1, 2, 4 and 6 microg/kg/min for 15 min each) with or without pretreatment with 10 mg p.o. bisoprolol and PRA, heart rate, contractility and blood pressure were assessed.

Dobutamine increased PRA more potently in Arg389 vs. Gly389 beta1-AR subjects. Bisoprolol markedly suppressed dobutamine-induced PRA-increase in Arg389 but only marginally in Gly389 beta1-AR subjects.

Dobutamine caused more pronounced heart rate- and contractility-increases and diastolic blood pressure-decreases in Arg389 vs. Gly389 beta1-AR subjects. Bisoprolol reduced dobutamine-induced heart rate- and contractility-increases and diastolic blood pressure-decreases more potently in Arg389 vs. Gly389 beta1-AR subjects.

Codon 389 beta1-AR polymorphism does not only determine hemodynamic but also PRA-effects. Thus, beta1-AR polymorphisms may be useful to predict therapeutic responses to betablocker treatment
