Abstract: Optic canal fracture is one of primary causes of hackneyed traumatic blindness.Differert schemes are recommended to treat impaired vision caused by traumatic optic canal fracture. It consists of observation,medical treatments to application of steroid and nerve decompression by different approaches. With the development of intranasal endoscopic operation and nose and eye surgery become gradually mature,intranasal endoscopic nerve decompression becomes possible. Smaller trauma, keen and indeed curative effection is the characteristic of the operation. In the operation, catagmatic fragments are removed in and around optic canal to lessen the oppression and injury of optic nerve,rev down pressure and improve local blood circulation,in the end elevate patients’ sight.

Objective: Based on the analysis of 55 patients to discuss the treatment methods and matters needing attention, to observe its clinical therapeutic effect.

Methods:55 eyes in 55 cases of traumatic optic nerve diseases(32 eyes with no light perception ),do the operation in 3-103 days after trauma with application of steroid before and after operation.Observe the recovery of vision after operation and sum up experience,

Results: In 25 of 55 cases visions access to improve,efficiency is 45%;among the total 20 patients get sight above 0.1, which means significant clinical effect.

Conclusion: For the patients with optic nerve damage caused by trauma, conservative treatment fails, we should be early and actively take surgical treatment.Recovery of vision depends on time after injury,degree and extent of optic nerve injury,Patients with no light perception and blinded immediately should either take this operation as early as possible.
