Objective: To investigate the evaluation on the preoperative virtual CT endoscopy of the congenital middle ear malformations and the selection of surgical approach.

Methods: Thirteen cases (16ears) with congenital middle ear malformations were underwent high resolution CT scanning to collect the original images data from the focus in middle ear, followed by 3-dimensional reconstruction to process these images for evaluating the possibly presented lesions with the ossicular chain, according to the level of the ossicular deformity, congenital middle ear malformations were divided into three types.Type A and C were selected auricular incision through attic pathway remaining intact canal wall;type B was selected traditional exploratory tympanotomy through ear canal incision.

Results: Virtual CT endoscopy observed that 5 ears were type A and 3 ears were type B, we selected auricular incision through attic pathway remaining intact canal wall.8 ears were type B,we selected vestibular window fenestration and artificial stapes surgery.Postoperative external auditory canal and tympanic membrane were still intact in 16 ears,and speech frequency hearing improved average of 25-45dB. The results of preoperative Virtual CT endoscopy were basically consistent with the surgical exploration.

Conclusions: Virtual CT endoscopy is an important reference for preoperative evaluation on congenital middle ear malformations. We can select different surgical approach according to the type of congenital middle ear malformations.
