Introduction: The term non Hodgkin lymphoma refers to a wide group of lymphoid neoplasms which affect extranodal sites in 20%-30%. 3-5% of them arise in the oral cavity and pharynx and most commonly involve the Waldeyer’s ring.

Case presentation: We present a 28 years old man, referred to our clinic for assessment of a formation at the base of the tongue. The patient had intermittent pain and progressive dysphagia dating back from six months. The tumor was diagnosed histopathologically and immunohistochemically as B-cell lymphoma. After 4 courses of chemotherapy was achieved a complete remission.

Conclusion: Non-Hodgkin’s lymphoma of the tongue base is a rare condition but it should always be considered, due to its specific treatment and prognosis, related to the stage of tumor and aggressiveness of the malignant cell type.

The fast diagnosis, based on proper visualization and imaging, deep tissue biopsy, thorough histopathotological assessment and immunohistochemical typing can drastically improve the prognosis of this serious disease.
