Objective: To evaluate the primary or recurrent cholesteatoma in children.

Design: Retrospective study.

Patients: 15 patients aged 13 to 18 years (mean age, 15 years) with cholesteatoma of the middle ear were surgically treatment and included for follow-up from 1 to 73 months between I 2004 and IX 2010. The early and late postoperative results were analyzed after the implemented reconstructive interventions.

Results: In all 15 children we used retroauricular skin incision and ant.-post. approach for following the matrix of cholesteatoma. During the operation in 12 cases was determinated the indication for second-look surgery.

Conclusions: Cholesteatoma in childhood is a very aggressive disease, that in most cases have indication for second-look surgery 1–2 years after the operation.
