Surgical treatment for facial skin tumours, no matter of their localisation, has two main purposes: excision within oncological limits and esthetic reconstruction of the affected area. Intraoperative anatomo-pathological examination avoids incomplete tumoral resection. In order to achieve an appropriate reconstruction of the facial defects there are different surgical techniques available. When selecting the most suitable technique the following factors must be taken into account: tumour location, the size of the facial defect, planning of the incisions along the skin tension lines to avoid deforming keloid scars. Even though the oncological safety limits are always respected, local reccurence is sometimes noticed probably due to the perineural invasion. Reconstructive surgery aims to accomplish a quite normal facial esthetic in order to offer an appropriate social life to the patient. The authors present their experience on a number of 123 patients who underwent surgery in the ENT Clinic from Cluj Napoca between 2000-2006. 112 (91.66%) patients were diagnosed with facial skin malignancies: 91 (74,5%) basal cell carcinomas, 23 (18.1%) squamous cell carcinomas and 9 (7.2%) malignant melanomas. Skin grafts, various local and regional skin flaps were used to reconstruct the facial defects following tumor resection. The outcomes are discussed by the authors.
