Background: Therapy of laryngeal carcinomas has changed over the last years. Localised stages of cancer are treated almost exclusively by laser-surgery. The larynx-preserving primary Radio-Chemotherapy is increasingly used in the treatment of advanced tumors.

Material and Methods: In a retrospective study from 1996 to 2006 all patients with the diagnosis of a laryngeal carcinoma have been evaluated regarding the overall survival. A total number of 260 patients (237 men and 23 women) with a mean age of 60 years was analysed. 53,2% of the patients had glottic and 26,4% supraglottic carcinomas.20,4 % of the cancers showed advanced stages with transglottic infiltration or extension beyond the larynx. At the time of diagnosis 49% of the tumors were distributed to stage T I–II and 51% to stage T III–IV.

Results: The 5 year overall survival rate added up to 92%, 70%, 62% and 40% in stage T I, II, III, IV, respectively. Peri- or postoperative complications have been recorded in 4,6% of the patients. Frequently these occurred in consequence to co-morbidity (metabolic syndrome, detoxication of alcohol). Recurrent disease occurred in 23,4% of the patients, half of it appeared as local relapse and regional/systemic metastases, respectively.

Conclusion: Prospective studies are needed to elucidate the potential of multimodal therapies to improve the overall survival of laryngeal carcinomas.
