Introduction: Friedman’s recent studies demonstrate that the results obtained by means of the classical UPPP technique are closely related to the staging of the obstructive sleep apnea (OSAS).

Material and method: Starting from Friedmann’s staging and relying on their own staging, the authors are trying to demonstrate its role in selecting the surgical technique used.

The study was applied on a number of pacients with OSAS, index AHI>10.

The system used was based on the size of tonsils, marked from 0 to 3, the position of bottom tongue, marked from 1 to 3 as well as the value of BMI weight index.

The existent conditions allowed the following surgical techniques to be used: UPPP and TE.

Discussion: The UPPP technique is most frequently used for stages I and II according to the size of tonsils and the position of the palate, while simple TE is most frequently applied in case of severe tonsil hypertrophy.

Conclusions: The positon of the palate and the size of tonsils are essential in the selection of the therapeutic technique used as well as in the evaluation of the post surgical results.
