Background: Timely revision of guidelines is important in order to bring new evidence into clinical practice quickly.
Context: By providing a Wikipedia-like enclosed website we aim to make new evidence available to clinicians. The website (wiKiNO) gives the opportunity to revise guidelines short cyclically. Also wiKiNO aims to take evidence based medicine (EBM) closer to clinical practice by inviting all otolaryngologists to participate.
Description of best practice: The Dutch Otolaryngology Association initiated this project in 2009. All otolaryngologists have access to the website and are invited to post new critically appraised topics (CATs). All CATs have the same structure and require a clinically relevant question, systematic literature research, summary of scientific findings, considerations and the answer to the question. There is a methodological editor and a group of content editors. The group of editors is stimulating the use of wiKiNO by organizing WiKiNO-workshops, awarding credit-points to otolaryngologists. Currently, the WiKiNO encompasses 300 approved CATs.
Lessons for guideline developers, adaptors, implementers, and/or users: The authors believe that WiKiNO is a very promising tool to update clinical guidelines short cyclically. To become a real success the content needs to be extended with more CATs covering all important otolaryngological topics. Involvement of residents is essential, because their effort makes the WiKiNO-system work.
