Bringing Ariadne into daily practice – implementation of guiding principles for patients with multimorbidity

Target group: clinical and epidemiological researchers, physicians, nurses.
Multimorbidity – the co-occurrence of several chronic diseases in a patient – is often overwhelming, both for patients and health care professionals. Disease specific medical guidelines are of limited value in the care for these patients. Challenges concern both medical, therapeutic considerations and tailoring medical decision making to patient preferences.
The aim of this workshop is to highlight different aspects of the complexity involved in person-centered care for people with multimorbidity and to bridge between the theoretical care model and daily routine in health care.
This workshop will:
- give an explanation of the Ariadne principles to support the management of patients with multimorbidity;
- present the first pilot results of the application of the Ariadne principles in Spanish primary care;
- present evidence mapping in the area of patient preferences;
- demonstrate and apply an example of measuring patient priorities.
Workshop format: short oral introductions, followed by small-group discussions focusing on methodological and clinical applications.
