This presentation will review indications for glaucoma surgery from two perspectives

a) failure to achieve a Target Pressure despite maximally tolerated medications

b) identification of a rate of visual field loss that will affect Quality of Life in the near future despite adequate medical treatment.

The former indication will follow from the results of initial therapy, without lengthy followup; the latter will use trend analysis of rate to assess future visual problems. This latter approach will emphsize the importance of the binocular field when making decisions.
