Objectives: Aim of this study was to compare early reoperation and revision rate of Journey II bi-cruciate stabilized (BCS) total knee system (TKS) and Journey I BCS TKS.

Methods: We conducted a retrospective longitudinal non-concurrent cohort study of 141 Journey II TKAs (average follow-up duration 1.8 years, from 0.1 to 3.9 years) in 132 patients (61.4% females, average age 65.0 years) performed from October 2012 to September 2016 and 155 Journey I TKAs (average follow-up duration 6.2 years, from 4.4 to 7.7 years) in 138 patients (58.0% females, average age 66.8 years) performed from January 2009 to July 2012. The primary outcome were complications requiring re-operation. Re-operations were classified into revisions (required removal or addition of device component) and other re-operations.

Results: There were 31 re-operations in 22 (2.28 per 100 OCY) TKAs in Journey I cohort and 5 re-operations in 4 TKAs (1.54 per 100 OCY) in Journey II cohort. The adjusted Cox regression Hazard Ratio (HR) was 3.50 (P=0.0257). Twenty-one revisions occurred in 16 (1.66 per 100 OCY) TKAs in Journey I cohort and 3 revision in 2 TKAs (0.77 per 100 OCY) in Journey II cohort. The adjusted HR was 3.90 (P= 0.0783) (Figure 1 [Fig. 1]).

Conclusions: The early revision rate for Journey II BCS is comparable to standard TKA systems. The re-operation and revision rates of Journey II BCS are lower compared with Journey I BCS TKA.
