Objective: Since 2001 we operated 65 patients using a Bryan prosthesis for cervical herniation treatment. This type of prosthesis should preserve the upper and lower discs from disc degeneration. Most of the patients presented soft disc herniation, but we also treated hard disc herniation and myelopathy.

25 patients, operated during the last 4 years, underwent MRI and cervical X Ray in flexion and extension.

In all 25 X Ray showed that the prosthesis were moving perfectly.

In 20 cases the MRI showed that the upper and lower discs were unchanged. Among these 20 patients, 15 presented normal discs in the preoperative investigation and 5 presented superior or inferior black discs.

In the remaining 5 cases (all black discs), the upper and lower discs showed a progression in the degeneration: 2 of them developed soft disc herniation in the lower disc; 1 showed a progression in the protrusion in the lower disc and the other 2 developed an osteophyte in the upper disc.

In conclusion we can say that Bryan prosthesis can help prevent disc degeneration in normal disc; in presence of a black superior or inferior disc, the cervical prosthesis cannot prevent a progression in degeneration.
