Objective: There is agreement that symptomatic sacral meningeal cyst with check-valve mechanism and/or large cysts which are space occupying lesions should be treated surgically. The purpose of this study is to determine indicators of the necessity of surgical intervention and its surgical techniques for the sacral meningeal cysts with check-valve mechanism.
Methods: In 10 patients presenting with sciatica, and neurological deficits, myelography, computed tomography (CT) myelography, and magnetic resonance Imaging (MRI) detected sacral meningeal cysts with check-valve mechanism. 1 patient had two primary cysts. 10 cysts were Type II cyst and 1 cyst was Type I cyst. Of the 10 cases, 9 were virgin cases and 1 was a recurrent case. For the 7 virgin cases (8 cysts) of Type II cyst, reefing was performed for the cyst, on the other hand, for the recurrent case, duraplasty and reefing were performed to eliminate the check-valve mechanism of the cyst. For the only 1 virgin case of Type II cyst, primary root was sacrificed because of huge cyst. For the Type I cyst, it was performed ligation of the neck of the cyst.
Results: In all the cases, the chief complaints disappeared immediately and no deteriorations of clinical symptoms after the operation with an average follow-up of 27 months.
Conclusions: With or without check-valve mechanism is very important to determine indicators of the necessity of surgical intervention for the sacral meningeal cysts.
