Experiences with endovascular therapy (Viabahn) of distal subclavian artery stenosis through open retrograde transbrachial access. A small case series

Einleitung: Distal stenosis of subclavian artery are a rare entity in patients with occlusive peripheral artery disease and difficult to treat. While the open approach is unfavourable due to operative damage to the surrounding anatomy, endovascular therapy may offer new options. We report here about the implantation of Viabahn stent-graft in distal stenosis of subclavian artery.
Material und Methoden: Through an open retrograde access Viabahn stent-grafts were deployed in Seldinger technique. 3 patients (4 arms) were treated. Follow-up was performed after 1 and 2 years.
Ergebnisse: Implantation was successful in all cases. After implantation patients were immediately started on aspirin (100 mg/d) and clopidogrel (75 mg/d) for at least 6 months. Follow-up showed a primary patency rate of 100% after 1 and 2 years.
Schlussfolgerung: The endovascular treatment of distal subclavian artery stenosis with Viabahn stent-graft exhibits good results after 1 and 2 years in a small case series. Large prospective studies are needed to assess its longterm patency.
