Introduction

Preoperative planning of TKA with correct mechanical leg axis requires an orientation of the components corresponding to the center of femoral head and ankle joint. Using conventional X-ray´s the accuracy of component alignment is not garanteed - postoperative analysis is difficult. We aimed to optimize the component-alignment by using a modified technique of x-ray-examination and computer-assisted planning. Furthermore we wanted to develop a method of postoperative analysis of component alignment.

Material and Methods

X-ray-examinations are done of the knee and additionally of the hip in frontal plane using special equipment. A self-developed-software allowes an alignment both of the femoral component corresponding to the center of femoral head as of the tibial component corresponding to the center of ankle joint. The analysis of postoperative X-ray's works in the same technique.

Results

The described method was used in 190 cases until today. It makes the preoperative planning of TKA more exactly, easily and rapidly. Comparison of preoperative planning, intraoperative situation and postoperative analysis has proved to be very useful for the operating surgeon.

Discussion

A new method of computer-assisted preoperative planning and postoperative analysis with conventional x-ray´s is described. It supports automatically the correct orientation of femoral and tibial components in preoperative planning of TKA. This is very useful for correction of mechanical leg axis. Furthermore it makes the preoperative planning of TKA more easily and rapidly. The need of an additional x-ray of the hip is the only disadvantage. We use the method routinely and recommend it.
