The aim of this prospective randomized trial was to compare the effect of two different weight reduction approaches with respect to the weight reduction, the metabolic burden and cardiovasculare risk factors in patients with morbide obesity (BMI >40 kg/m2).
Methods
Weight reduction approaches- surgical with gastric banding (gb) vs. conservative (cons: energy/fat reduced diet - 600 kcal/d, 1,5 hour aquafitness/week, behavioural therapy (10 sessions) and 10 mg Sibutramine/d.
Results
Results are listened in table 1 [Tab. 1].
The patients of the gb-group had a higher initial weight (152 vs 131 kg) and a insignificant worse basic metabolic situation (Insulin Sensivity IS [Avignon] 0,59 vs 0,86 108mg-1mU-1ml). But after 12 month the cons-group had shown significant better results regarding the metabolic situation (IS 1,16 vs 2,39 108mg-1mU-1ml).
For the first time we could show that it is possible to reduce weight in patients with extremely obesity with a conservative approache to a same external like after gastric banding. Despite the use of a morepinephrine and serotonin reuptake inhibitor we couldn't detect any differences in respect of blood pressure (ABPM, 100 Watt ergometry) and heart rate. The cardiovascular risk profile decrease in both groups with a tendency to better metabolic results in conservative (pharmacology) treated patients.
