Objective: To investigate the nasal septum perforation perioperative principles and techniques.

Methods: A retrospective summary of 38 cases of nasal septum perforation surgery, including preoperative treatment, the choice of different surgical approaches, the choice of different surgical methods, postoperative treatment, which include 10 cases of anterior nostril, nasal alae cut in 4 cases, columella butterfly cut in 2 cases, endoscopic path in18 cases, lapial sulcus path in 2 cases, 2 cases of midface overturned path.

Results: The all patients (38 cases)were follow-up from January to June, only 1 perforation did not heal due to lack of experience and were healed by re-operation.

Conclusions: The success of the operation determined the perioperative each session, the surgeon should following minimally invasive, efficient and aesthetic principles to choose a different surgical approach and repair methods according to the size of perforation, location, structural characteristics of the nasal cavity. The complex tissue flap combined with heterogeneous (cow) acellular dermal matrix by endoscopic can increase the success rate in repairing the nasal septal perforation, surgeon's experience and to keep the nose moist is essential.
