Supracricoid hemipharyngo-laryngectomy for treatment of T1-2 hypopharyngeal tumors

Introduction: Authors report function spearing surgical treatment – supracricoid hemipharyngo-laryngectomy – for the treatment tumors of the piriform sinus. Surgical technic, oncological and functional result are demonstrated .
Method: 25 patients were retrospectively reviewed between 2000 and 2005 who underwent supracricoid hemipharyngo-laryngectomy with neck dissection for hypopharyngeal squamous cell carcinoma and neck metastasis in our department. Our surgical method contained removal of the half of the hyoid bone, the prelaryngeal muscles are unilateraly dissected and resection the lateral part of the thyroid cartilige and infiltrated pyriform sinus together with ary-region, above the level of the cricoid cartilige. The perichondrium of resected thyroidal cartilage was preserved and for the reconstruction of the neopharynx beside the pharyngeal mucosa and muscle was also applied. Prelaryngeal muscles were also reconstructed. 12 patients had postoperativ irradiation.
Results: Of the 25 patients 12 have no sign of recurrence (48%), 5 patients had recurrent metastasis (20%) required sRND, 2 patients had local recurrance and required total laryngectomy, in 2 patients observed second primary tumor (8%), 2 patients had distant metastasis (paratracheal-mediastinal, inguinal), 1 patients died of local recurrance, 1 patients died of general conditional problem.
Conclusion: Authors consider the method accounted a suitable function maintaing treatment in selective cases of piriform sinus cancer.
