The application of the titanium prostheses in the middle ear surgery was chosen as a means for the reconstruction of the ossicular chain.

Occasionally, during necessary revision surgeries it happens that the prosthesis is pulled down caused by a scar formation of the ossicular chain, that there is no contact with the tympanic membrane transplant or that there is a protrusion. In this case the prostheses which possess a clip mechanism and a hinge for a better adjustment to the reconstructed tympanic membrane transplant may have advantages. It should be investigated whether the reduction of postoperative ABG in comparison with conventional prostheses through the application of CliP Partial FlexiBAL prostheses is feasible and if there is any difference regarding any complications. Since May 2010 50 patients were equipped with a CliP Partial FlexiBAL prosthesis (from 2.25 mm to 2.5 mm) at the ENT-Clinic-Halberstadt. A group of 50 patients who were equipped with Titanium Partial Prostheses without clip mechanism served as a comparison group for the reconstruction of the ossicular chain. The postoperative bone conduction threshold was evaluated regarding the risks of inner ear traumatization through the clip mechanism. For the evaluation of the postoperative results the reduction of the ABG was compared. A suspected damage to the inner ear caused by the clip mechanism was not determined; on the contrary, the clip prostheses showed better results than conventional partial prostheses. A significant advantage of the clip prostheses regarding the reduction of the ABG could not be observed. In the cases of 5 patients a revision had to be carried out because of a relapse defect. Within the comparison group in the cases of 8 patients a revision was necessary.
