The therapy of choice for nasal septum perforations is operational treatment in Germany. A reduced chance of success by unfavorable local and general conditions or the refusal of the patient can be reasons for the renouncement of an operational treatment. As an alternative the supply of silicon buttons is applicable.

From 2009 to 2010 we supplied 19 patients with individually made silicon buttons with two different degrees of hardness and a non-adhesive coating. The technique of casting the septum defect, described by Federspil and Schneider 2006, was modified and simplified, so that the casting could always be taken in local anaesthesia and was successful at the very first attemp. 6 month later all patients showed up with the silicon button still in situ and without any problems. The silicon buttons fitted so perfectly that the reduction rate of symptoms and complaints, as pain, incrustation and epistaxis was 100%! The durability of the non-adhesive coating of the silicon buttons, which reduce the incrustation of the nose clearly in interaction with daily nose flushings, is still observed.

The supply of the perforations of the nasal septum with individually made silicon buttons is a simple and safe method to reduce the complaints of these patients significantly.
