Sarcomas are malignant tumors, which occur everywhere in the body. They occur with a frequency of 2 new cases by 100,000 inhabitants/year.

A genetically disposition is not known but an irradiation applies as a secured risk factor. Patients with an irradiation dose ≥50 Gy have a higher risc to fall ill with a second cancer. Often the diagnose “sarcoma” is made in an advanced stage, because the tumor grows infiltrativ but slow.

We report on two cases in which patients get a sarcoma within the radiation field. In both cases it was diagnosed in the cancer follow-up investigation, one after 7, the other after 13 years. Both cancers were treated with an endoscopic laser surgical ablation. Previously no recurrence could be demonstrated.

A long-termed and periodic monitoring of irradiated patients is very important because of late genesis of second cancers within the radiation field. With this periodic monitoring is it possible to detect and therapy some patients with a recurrence before they notice it.
